Responsibilities
*
The pharmacist will provide integrated pharmaceutical care through integrated drug distribution, clinical services, as well as patient and provider education

* Be an effective communicator
* Provide accurate electronic order entry of medication orders
* Perform accurate verification of all electronically prescribed orders for possible therapeutic problems, contraindications, interactions, and allergies; the pharmacist will collaborate with the prescriber to promptly resolve these order verification problems
* Ensure the appropriateness of medication orders relative to the patient's level of care and unit
* Ensure department compliance with controlled substance distribution and regulatory system, including checkout and receipt of anesthesia boxes, verification of all controlled substance ADM refills, and verifying the appropriate sign-out of controlled drips
* Accurately verify the correct refilling of EMS (Health Emergency Medical Services) medication boxes, A-Packs (accessory packs), and crash carts
* Verify the filling of home medications prior to dispensing on cart fills
* Check all pharmacy bulk solid and liquid dosage form repackaging for appropriateness
* Check unit-dose medications for accuracy prior to dispensing or filling into automated dispensing machines (ADMs)
* Check IV medication preparations for accuracy and labeling appropriateness as well as beyond-use-dating based on stability from time of preparation
* Ensure the timely dispensing of medication orders from central pharmacy, especially in emergency situations
* Act as a steward for formulary compliance and work closely with the clinical manager to provide prescribers with evidence-based recommendations for alternatives
* Participate in continuing education programs, meetings, training programs and related activities
* Maintain competence with all the department's initiatives and programs
* Perform medication histories and regimen adherence reviews and discharge patient education as required
* Perform accurate, high-quality, and complete documentation of all clinical activities within the electronic medical record
* Complete prospective medication regimen reviews on all patients on their respective floor(s)/unit(s)
* Promptly and accurately complete dosing consults as delineated by hospital/health-system policy and effectively communicate the completion of such activity to the patient's treatment team as required
* Work with the home medication technician to answer questions and expedite the resolution of all home medication related concerns prior to the end of their shift
* Concurrently monitor use of drugs for compliance with prescribing criteria, guidelines and protocols
* Provide complete and appropriate verbal and electronic handoffs to the incoming pharmacists or to pharmacists on the same shift
* Be familiar with the hospital and health-system's policies directly correlated to their daily practice
* Precept Introductory Pharmacy Practice Experience (IPPE) and Advanced Pharmacy Practice Experience (APPE) rotation students as required

* This shift is rotating days and afternoons; some midnights as needed and every other weekend

Qualifications:
* Must hold a degree from an accredited college of pharmacy and should have served an internship and/or residency in hospital pharmacy.
* Must be licensed or eligible for licensure in the State of Michigan.
* Has earned a Doctor in pharmacy (PharmD) degree
* Has completed an ASHP-accredited post-graduate residency (PGY1) or has equivalent practice experience (2 to 3 years of full-time inpatient clinical practice)
* Has a clean practice record
